This central versus peripheral distinction is the defining mechanistic feature that has driven pharmaceutical development in this space, enabling therapy in conditions where PDE5 inhibitors fail — including psychogenic sexual dysfunction with intact vascular function, and (importantly) female sexual dysfunction where PDE5 pharmacology has limited applicability due to the different vascular architecture of female sexual response. The central-arousal mechanism opens therapeutic space that was pharmacologically intractable before melanocortin research matured. PT-141 (bremelanotide, marketed as Vyleesi) is the pharmacological centerpiece of the category. Developed by Palatin Technologies through systematic optimisation of the earlier Melanotan II template, PT-141 is a cyclic heptapeptide (Ac-Nle-cyclo[Asp-His-D-Phe-Arg-Trp-Lys]-OH) with retained agonism at MC3R, MC4R, and MC5R and reduced (but not eliminated) MC1R activity — the medicinal-chemistry rationale being to preserve the central sexual-function effects while reducing the skin-pigmentation effects that would complicate cosmetic acceptability. The principal pharmacology is MC4R activation in hypothalamic paraventricular nucleus, medial preoptic area, and brainstem sexual-function circuits including the periaqueductal gray, producing sexual arousal and libido effects mediated by dopaminergic release in reward-and-arousal circuits, oxytocin release, and modulation of sexual-behaviour neural pathways. FDA approval as Vyleesi came in June 2019 for hypoactive sexual desire disorder (HSDD) in premenopausal women — the first melanocortin-receptor agonist approved for a sexual-function indication. Vyleesi is administered on-demand subcutaneously (via pre-filled autoinjector) 45 minutes to a few hours before anticipated sexual activity, maximum 8 doses per month. NOT MHRA-licensed in the UK. Melanotan II (Ac-Nle-cyclo[Asp-His-D-Phe-Arg-Trp-Lys]-NH2) is the earlier University of Arizona-developed cyclic heptapeptide from which PT-141 was optimised. Developed in the 1980s as a synthetic α-MSH analogue for skin pigmentation research, its clinical exploration surfaced the unexpected sexual-arousal effects that motivated the Palatin PT-141 development programme. Melanotan II remains non-selective across the melanocortin receptors (MC1R skin pigmentation, MC3R/MC4R central appetite and sexual function, MC5R exocrine). The critical safety concern with Melanotan II is its effect on existing melanocytic naevi (moles) — MC1R activation drives melanogenesis in all melanocytes including those in existing naevi, producing changes that can mimic melanoma progression. Case reports of atypical mole changes, dysplastic naevi development, and rare melanoma diagnoses have accumulated in dermatology literature. The MHRA has issued multiple explicit public warnings against Melanotan II use. It holds no marketing authorisation in any jurisdiction. KPV, mechanistically related through α-MSH ancestry, is included in cross-references but acts principally through NF-κB pathway modulation rather than melanocortin-receptor engagement — mechanistically it belongs in the anti-inflammatory rather than sexual-health category. Pre-clinical melanocortin pharmacology assays include cAMP accumulation in receptor-expressing cell lines (HEK293 or CHO cells transfected with individual MC receptors); competitive binding studies against radiolabelled α-MSH; behavioural sexual-function assessment in rodent models (paced-mating paradigms, sexual solicitation frequency, hesitation/rejection response scoring); food intake and body weight measurements in appetite research; and skin pigmentation measurement (melanin content, standardised photography, Fontana-Masson staining) for MC1R-focused pigmentation research. UK regulatory positioning of this category is unusual in the healing-peptides context. PT-141 (Vyleesi) has FDA approval but NOT MHRA — specialist access through the unlicensed-medicines 'specials' route via US pharmacies is theoretically possible for individual HSDD patients but rarely pursued. Melanotan II is explicitly MHRA-warned; supply for human use is generally an offence under the Human Medicines Regulations 2012, with recurring MHRA enforcement actions against suppliers. Open research questions include whether the modest cardiovascular effects of PT-141 (transient BP/heart rate elevation) preclude broader male erectile dysfunction indications where PDE5 inhibitors dominate; the mechanistic basis of interindividual variability in melanocortin-driven sexual arousal effects; and whether medicinal-chemistry optimisation can produce future melanocortin analogues with even further-reduced MC1R activity eliminating the pigmentation concern that limits Melanotan II use.
